Requires a physician referral and needs to be based on specific medical diagnosis may have 100% coverage, but some plans do requires deductible to be met.
BLUE CROSS BLUE SHIELD/ANTHEM
Most plans have full coverage with no referral, no deductible, no co-pay, and no out of pocket and no visit limit.
Many of these plans pay 100% with no deductible, no referral, but some do have limited number of visits.
Usually requires a medical diagnosis, may only have 1-3 visits covered annually.
Must have a diabetes diagnosis or renal disease (not on dialysis).
Aetna Considers nutritional counseling a medically necessary preventive service for children and adults who are obese (BMI over 30), and for adults who are overweight (BMI between 25-29.9) and have other cardiovascular disease risk factors (hypertension, dyslipidemia, impaired fasting glucose, or the metabolic syndrome), when it is prescribed by a physician and furnished by a registered dietitian nutritionist.
Main medical conditions that have nutrition intervention:
(i) Cardiovascular disease, (ii) Diabetes mellitus, (iii) Hypertension, (iv) Kidney disease, (v) Eating
disorders, (vi) Gastrointestinal disorders, (vii) Seizures (i.e., ketogenic diet), and other conditions
(e.g., chronic obstructive pulmonary disease) based on the efficacy of diet and lifestyle on the
treatment of these diseased states.

Our visits can be as short as 15 minutes or as long as you need. Pricing is based on $25 per each 15 minutes.
